Shift: 8:00 AM - 4:30 PM

Experience:

12 months of experience with 3 months worked in the last 12 months.

12 months of Home Health experience.

Experience with Oasis is required.

Requirements:

Candidates must have a license (required after submission).

This role may require floating to additional units and locations

Local & travel allowed. Candidates living Additional Details:

Required Skills / Experience: 1 year of Skilled Home Health Experience

Preferred Skills / Experience: Acute Disorder, Chronic Disorder, Functional Activities

Unit / Facility Details: May visit up to 6 patients per day

Traveler is expected to have their own transportation. System does a good job grouping clients minimizing drive and windshield time. Occasional maximum radius of roughly 60 miles (60-90 minute total drive time) per day.

Special Requests: RTO must be listed at time of submission.

All positions at Providence Health & Services require that any and all lab items requested during onboarding be completed within 72 hours of signing a contract - no exceptions. This means candidates must be responsive and willing to go to a lab to complete health orders ASAP after signing. Failure to comply may lead to a pushed start or cancellation as assessed by the facility.

References must cover at least 1 full year of relevant experience in specialty within the past 3 years

Cannot have any hits/suspensions on license or any background hits from any point in the past.

You must be able to complete labs within 48 hours of signing.

Cannot be currently or recently (within 6 months) employed with network - Must have left in good status, if former employee.

ER profiles must include the size of the facility / # beds and the Trauma Level designation (if any)

Orientation for all Oregon facilities is in Portland, OR for RN, LPN and CNAs.

Shift & Scheduling: No Weekend, Call

Floating Requirement: Hood River, OR and Surrounding Areas

5 days maximum time off

2 Manager/Supervisor references required from within the last 3 years

Proof of identification required
