Responsible for the planning and implemention of Speech-Language Phology evaluion and trement services. Evalues communicion disorders and administers therapy trements for resident/pient problems associed with hearing, speech, language, cognitive and swallowing disorders. Maintains records, monitors progress, and counsels residents or pients and their family members.

Core Job Responsibilities
  • Responsible for administrion and direct applicion of Speech/Language Phology trement modalities as prescribed by a referring physician.
  • Evalues, assesses, plans, directs and tres all residents or pients referred for speech/language phology services. Interprets test results in accordance with standard speech/language procedures.
  • Re-evalues pients as approprie. Implements new trement plans and goals as necessary.
  • Implements trements to restore function and prevent disability following disease or injury.
  • Participes in multidisciplinary consultions.
  • Participes in performance improvement activities within the department and participes in CQI activities.
  • Completes continuing educion and in-service programs to meet regulory and job requirements as applicable.
  • May provide informion reled to departmental budget considerions in relionship to Speech Phology needs for optimum pient care and service.
  • May assists in scheduling pient or resident visits for further evaluion and trement.
  • Ensures all documention and charts are complete.
  • Participes in infection control, safety and educional programs, both in the department and on an organizional wide basis.
  • Performs other duties as assigned.
Educion/Experience Requirements

REQUIRED:

  • Gradue of a college/university accredited by the American Speech and Hearing Associion with a degree in Speech Language Phology.
  • Excellent communicion skills to communice with pients, medical and house staff. Excellent assessment skills to make accure prognosis of pient potential and guide the individual to achieve th potential.
  • Proficient with MS Office and Epic\EMR software.

PREFERRED:

  • Prior experience in a long-term care facility working as a Speech- Language Phology (SLP)
Licensure/Certificion Requirements

REQUIRED:

  • Current NYS license as a Speech-Language Phologist (or temporary permit).
  • Must hold or be eligible for the Certifice of Clinical Competence in Speech Language Phology from the American Speech, Language and Hearing Associion (ASHA).
