Salary: $35,000 $40,000 annually
We are seeking a compassionate and detail-oriented Targeted Case Manager (TCM) / Supportive Housing Specialist to work with individuals facing Serious Emotional Disturbance (SED), Severe Mental Illness (SMI), Substance Use Disorder (SUD), and Co-occurring Disorders. You will help clients access services, develop treatment plans, and maintain stable housing for maximum independence.
What Youll Do:Provide assessment, treatment planning, advocacy, support, and service coordination for assigned clients.
Assist participants with housing-related activities to promote community integration and independence.
Ensure services comply with Kentucky Department of Behavioral Health and Medicaid guidelines.
Document client progress and maintain electronic medical records accurately.
Collaborate with team members and community partners to ensure comprehensive care.
Bachelors Degree in Human Services (from approved KAR list).
One year of experience working with populations served (including practicum/internship).
Completion of 12-hour Core TCM training and specialized population training (SED, SMI, SUD, Co-occurring) within 6 months of hire, or valid TCM certification from a previous employer.
Proficient with Electronic Medical Records, Microsoft 365, and Excel.
Strong communication skills and ability to work well in a team.
Ability to adapt to change and follow agency policies and state regulations.
