Salary: $35,000 $40,000 annually

Position Overview:

We are seeking a compassionate and detail-oriented Targeted Case Manager (TCM) / Supportive Housing Specialist to work with individuals facing Serious Emotional Disturbance (SED), Severe Mental Illness (SMI), Substance Use Disorder (SUD), and Co-occurring Disorders. You will help clients access services, develop treatment plans, and maintain stable housing for maximum independence.

What Youll Do:
  • Provide assessment, treatment planning, advocacy, support, and service coordination for assigned clients.

  • Assist participants with housing-related activities to promote community integration and independence.

  • Ensure services comply with Kentucky Department of Behavioral Health and Medicaid guidelines.

  • Document client progress and maintain electronic medical records accurately.

  • Collaborate with team members and community partners to ensure comprehensive care.

Requirements:
  • Bachelors Degree in Human Services (from approved KAR list).

  • One year of experience working with populations served (including practicum/internship).

  • Completion of 12-hour Core TCM training and specialized population training (SED, SMI, SUD, Co-occurring) within 6 months of hire, or valid TCM certification from a previous employer.

  • Proficient with Electronic Medical Records, Microsoft 365, and Excel.

  • Strong communication skills and ability to work well in a team.

  • Ability to adapt to change and follow agency policies and state regulations.
