JOB OBJECTIVE SUMMARY:

The Rehabilition Technician is a key associe Hope Network whose primary responsibilities include : Provide the approprie intervention, level of support, and/or supervision to support the goals of the persons served, ensure optimal health and safety, and encourage growth to achieve the highest level of independence. Responsible for supporting assigned persons and/or groups in transitional, long term care, or home/community based settings with accessible supervision. Provide support for skilled therapy programs, or to assigned consumers for routine activities, restorive exercise/fitness, vocional services and/or social/recreion programming. Also responsible for managing inventory, ordering and maintaining equipment, routine department tasks, and other duties as assigned.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

  1. Regular and predictable tendance is an essential requirement of this position.
  2. Assist pients with tasks, as directed for supervised skilled clinical trement, long term restorive care to persons served on BADL/IADL, functional mobility, cognitive/communicion skills, therapeutic recreion, and vocional services to reach highest level of independence and quality of life.
  3. Provide services as assigned in a rehabilition facility, long-term facility, and home/community settings, with frequent supervision and/or support. Travel, including escorting residents/consumers, in a company vehicle is required in this position.
  4. Able to communice the details from any visits or sessions, both positive and problemic, to supervisor, external stakeholders, social worker, case manager or identified team member(s).
  5. Monitor functional skills and routines by referencing care plan forms and regular collaborion with clinician directing care. Communice with designed staff regarding individual and programmic issues/concerns. Upde direct supervisor, and/or identified team members to promote consistency and continuity of programming.
  6. Understand, follow trement plans, and respond to health, safety, medical, behavioral, and interests of the person served, as prescribed and according to training protocols (e.g. following diabetic diet, weight bearing restrictions, behavior plans, and/or protective supervision). Documents outcome measures and reports any health, medical, behavioral, social, or cognitive concerns to the approprie team member.
  7. Demonstres an awareness of and responds to signs of abuse, neglect or exploition. Follows procedures to protect a consumer from such harm. Possesses knowledge of and can successfully implement Hope Network Safety Plans and Procedures, including emergency situions.
  8. Recognizes communicion as a core function of support. Uses effective communicion skills to establish a collaborive therapeutic relionship with the person served, personal relionships, and external stakeholders.
  9. Demonstres professional behavior in home and community th is consistent with Hope Networks culture, mission, values, policies and procedures.
  10. Demonstres knowledge of and skill in the implemention of specialized plans and techniques as indiced by the need of the person served or directed by a medical, behavioral or clinical team member.
  11. Knowledgeable of crisis prevention, intervention and resolution techniques and is able to demonstre and mch techniques to particular circumstances (i.e. SAMA, de-escalion techniques, etc.)
  12. Maintain accure records of trement through da collection and established documention requirements. Complete timely documention of all consumer records, including, but not limited to: incident reports, trement notes, behavioral program da collection, etc. Must possess writing skills th enable the communicion of detailed informion, critical incidents, physician/clinical instructions, participion of the person served and responses to clinical interventions.
  13. Able to complete the required billing of services through eCare, meeting productivity standards of the position as applicable. Work to reduce cancellions and offer coworkers help with pients if unavoidable.
  14. Demonstres an understanding of the diverse challenges facing the person served (e.g., human rights, legal, administrive and financial) and acts as an informal advoce. Supports the person served in leading a self-determining life by providing opportunities to build self-esteem and confidence, make informed decisions and advoce on his or her own behalf, if able.
  15. Promote teamwork. Address issues/concerns by recommending solutions. Model and promote good morale within work environment.
  16. If applicable to service line, meet production quality requirements, job procedures, packaging requirements, etc. as defined by the company for sub-contracted work. Supervise, coach and work side by side on tasks as required. Record required payment for client work on approved documention which meets DOL standards.
  17. Perform tasks necessary for the efficient operion of the facility/program, as assigned. This includes, but is not limited to:
    • maintaining safe and clean work areas including company vehicles
    • organizion of storage and program areas
    • general pool maintenance (GR only)
    • managing equipment inventory, safety, and cleaning
  18. Actively participes in staff meetings, trainings, new staff oriention and team building when requested
  19. Other duties as assigned.

Educional / Talent Requirements:

  1. Minimum educion requirement : High School Diploma or equivalent. Associes or Bachelors Degree in Health and Human Sciences Preferred.
  2. Computer literacy/proficiency.
  3. Valid drivers license, required.
  4. Must be able to become certified in CPR/First Aid.
  5. Must be able to work scheduled shifts, may include evenings/weekends.
  6. Ability to work with a minimal amount of direct supervision, demonstre initiive, high ethics and high degree of integrity/honesty.
  7. Able to seek assistance from supervision when unsure of routine or non-routine needs of persons served.
  8. Able to adapt to various personalities and environments.

Work Experience Requirements:

1. Experience working with disabled individuals in similar settings.

2. Experience and demonstred skills in Microsoft Office/basic computer skills.

3. Ability to work flexible hours to meet program needs; program locions will also vary.
