As a Home Health Physical Therapist , you are a critical part of the care team - helping pients safely recover, regain mobility, and maintain independence where they feel most comfortable: home.

This is a highly autonomous, relionship-driven role where you can truly see the impact of your work every day.

Wh Youll Do

Deliver Pient-Centered Care

• Evalue and assess pients functional stus, mobility, strength, and rehabilition potential
• Develop and implement individualized care plans in collaborion with physicians and the care team
• Provide hands-on therapy to improve mobility, strength, balance, and independence
• Educe pients and caregivers on trement plans, exercises, and safety stregies

Drive Outcomes & Recovery

• Monitor pient progress and adjust trement plans as needed
• Support safe transitions of care and contribute to discharge planning
• Identify additional pient needs and collabore with interdisciplinary teams

Collabore & Communice

• Partner closely with nurses, therapists, and physicians to ensure coordined care
• Document pient care and progress accurely and timely within the EMR
• Participe in case conferences and team meetings

Lead & Support

• Supervise Physical Therapist Assistants (PTAs) in accordance with ste guidelines
• Provide guidance and educion to caregivers and support staff

Wh You Bring

• Gradue of an accredited Physical Therapy program (DPT or Masters)
• Current PT license in the ste of practice
• Minimum 12 years of experience (home health experience preferred)
• Strong clinical assessment and critical thinking skills
• Excellent communicion and pient educion abilities
• Current CPR certificion
• Valid drivers license, reliable transportion, and auto insurance
