Home Health Physical Therapist (PT) in Lady Lake, FL

In this contract opportunity, youll play a vital role providing in-home physical therapy services, supporting individuals as they recover, regain independence, and improve quality of life.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract

Qualifications:

• Valid and active Physical Therapist (PT) license in the state of practice
• Minimum of 1 year recent experience in Home Health Physical Therapy, with field-based experience preferred
• Competency in Home Care Home Base (HCHB) strongly preferred
• Proficiency conducting OASIS assessments is required
• Solid clinical judgment and effective communication abilities
• Reliable transportation, current drivers license, and up-to-date auto insurance
• Strong organizational skills and ability to work independently in the field

Responsibilities:

• Complete comprehensive patient evaluations and OASIS assessments as needed
• Develop, implement, and update individualized plans of care to address mobility, strength, balance, safety, and independence
• Deliver skilled interventions: therapeutic exercises, gait and transfer training, neuromuscular re-education, pain management, fall prevention, and home safety education
• Educate and support patients and caregivers with use of assistive devices and home exercise programs
• Maintain accurate, timely electronic documentation through HCHB
• Collaborate with nurses and allied health professionals, participating in case conferences and interdisciplinary meetings to ensure coordinated care
• Comply with agency policies and Medicare home health regulations
