Home Health Physical Therapist in Leesburg, FL

Deliver high-quality, patient-centered care as a Home Health Physical Therapist, providing skilled services to individuals in their homes. This contract opportunity is well-suited to PTs who are self-motivated, organized, and comfortable working independently in a field-based setting. You will play a key role in improving mobility, functional independence, and quality of life for patients while collaborating with an interdisciplinary team.

Schedule & Assignment

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract
• Expected caseload of 2532 points per week (57 patient visits per day)
• Daily travel within assigned territory (mileage reimbursement provided)

Key Responsibilities

• Complete comprehensive physical therapy evaluations, including OASIS assessments
• Design and implement individualized care plans with focus on functional outcomes: mobility, strength, balance, and safety
• Deliver skilled interventions (therapeutic exercises, gait/transfer training, neuromuscular re-education, fall prevention)
• Monitor and communicate patient progress, adjusting plans of care as needed
• Educate and instruct patients and caregivers on exercises, safety, and assistive devices
• Document accurately and promptly using Home Care Home Base (HCHB)
• Coordinate with nurses, OTs, SLPs, social workers, and home health aides
• Participate in meetings, case conferences, and ongoing education
• Remain compliant with agency policies, regulatory standards, and Medicare guidelines

Qualifications

• Current, active Physical Therapist (PT) license (state of practice)
• Minimum 1 year recent experience in home health physical therapy
• OASIS experience required; HCHB proficiency preferred
• Strong clinical assessment and communication skills
• Reliable transportation, valid drivers license, and auto insurance
• Ability to manage a field-based, flexible schedule independently
