Home Health Physical Therapist (PT) - Eustis, FL

Make a direct impact on patient mobility and independence as a skilled Home Health Physical Therapist. This contract role centers on delivering individualized, high-quality care directly to patients in their homes, promoting functional improvements in mobility, strength, and overall daily living. Success in this field-based position requires strong clinical initiative, well-honed assessment skills, and a commitment to patient-focused outcomes.

Schedule:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM

What Youll Do:

• Complete comprehensive physical therapy evaluations, including required OASIS assessments for Start of Care, Resumption, Recertification, and Discharge.
• Develop, implement, and adjust individualized care plans targeting functional mobility, balance, gait, pain reduction, and patient safety.
• Deliver skilled PT interventions such as therapeutic exercise, neuromuscular re-education, gait and transfer training, and fall prevention.
• Educate patients and caregivers on exercises, safety, assistive device use, and home programs to support progress beyond each visit.
• Maintain timely, accurate documentation using Home Care Home Base (HCHB).
• Collaborate with fellow home health professionalsnurses, OTs, SLPs, social workers, and aidesto coordinate care and attend interdisciplinary meetings.
• Ensure compliance with agency policies, regulatory standards, and Medicare home health guidelines.
• Perform daily travel within the assigned territory with mileage reimbursement provided.

Your Experience & Qualifications:

• Active Physical Therapist (PT) license in good standing in the state of practice.
• At least one year of recent home health physical therapy experience (field care strongly preferred).
• OASIS assessment experience required; proficiency with HCHB preferred.
• Excellent communication, organization, and independent time management.
• Valid drivers license, reliable transportation, and auto insurance.
