Minimum Requirements

  • Bachelors, Masters, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
  • Active Indiana physical therapy license required to start the assignment.
  • BLS certification from the American Heart Association or American Red Cross may be required.
  • Experience working in home health or outpatient settings preferred.
  • Valid drivers license and reliable transportation required for travel between patient homes.

Preferred Qualifications

  • Experience with musculoskeletal and post-operative rehabilitation preferred.
  • Familiarity with documentation systems and electronic medical records preferred.

Assignment Details

  • Shift Days: M-F
  • Shift Hours: 8a-5p
  • Position requires travel between patient residences within the service area.
