Wh Youll Do

Deliver High-Quality Pient Care

• Provide direct physical therapy services under the direction of a licensed Physical Therapist
• Follow individualized plans of care to help pients reduce pain, improve mobility, and restore function
• Assist pients with therapeutic exercises, gait training, balance, and functional mobility
• Support pients with transfers, strengthening, and activities of daily living
• Utilize therapeutic modalities such as he, cold, ultrasound, and electrical stimulion as approprie
• Educe pients and caregivers on exercises, safety, and use of assistive devices

Monitor Progress & Communice

• Observe and report pient progress, response to trement, and any changes in condition
• Maintain accure, timely documention in the EMR
• Communice effectively with the Physical Therapist and interdisciplinary care team
• Participe in case conferences and care coordinion efforts

Collabore & Contribute

• Work closely with nurses, therapists, and other healthcare professionals to ensure coordined care
• Participe in team meetings, in-services, and ongoing training
• Uphold clinical standards and contribute to quality improvement efforts

Wh You Bring

• Gradue of an accredited Physical Therapy Assistant (PTA) program
• Current PTA license or certificion as required by ste law
• 1+ year of experience preferred (home health experience a plus)
• Current CPR certificion
• Valid drivers license, reliable transportion, and auto insurance
