Physical Therapist Outpatient Fort Collins, CO
Provide evaluations and ongoing physical therapy treatment for outpatient populations. Develop, implement, and modify individualized treatment plans based on patient needs, goals, and physician direction. Delegate and direct discipline-specific therapy services for assigned patients. Complete timely, accurate documentation including evaluations, goals, progress notes, letters of medical necessity, and charge entry in compliance with state, federal and payer requirements. Participate in student supervision as assigned. Maintain professional licensure and comply with state practice acts and organizational policies. Follow established safety and infection control practices.
Minimum Requirements
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program.
- Active Colorado physical therapy license required to start the assignment.
- BLS certification from AHA or ARC may be required.
- Experience in outpatient physical therapy settings preferred, including 60-minute initial evaluations and 30-minute follow-up visits.
- Demonstrated competency in documentation, goal writing, and letters of medical necessity.
- Ability to participate in student clinical education when assigned.
Preferred Qualifications
- Experience with outpatient program delivery and functional treatment planning focused on return to community activities.
- Previous experience with performance improvement initiatives and interdisciplinary collaboration.
Assignment Details
- Schedule: 5x8.
- Clinic expects 60-minute evaluations and 30-minute follow-up visits.
- Candidate may be expected to follow clinic guidelines for appearance, attendance, and punctuality.
