Physical Therapist Travel | Pulaski, TN | Home Health
Provide skilled physical therapy services in patients homes according to physician orders. Perform patient evaluations, develop and update individualized plans of care, document progress, and communicate changes in patient condition to the interdisciplinary team. Supervise and evaluate Physical Therapy Assistants and Home Health Aides as assigned. Evaluate home environments, make safety and equipment recommendations, and participate in in-service education. Must have reliable transportation, a valid drivers license, and agency-required liability insurance.
Minimum Qualifications
- Bachelors, Masters, or Doctoral degree in Physical Therapy from an accredited program approved by the American Physical Therapy Association or equivalent accreditation body.
- Current, unencumbered license to practice as a Physical Therapist in the state of Tennessee prior to the start of the assignment.
- Current CPR certification.
- Familiarity with electronic documentation and basic computer functions, including email and online training platforms.
- Must have reliable transportation and a valid drivers license.
- Ability to supervise and evaluate PTAs and home health aides as assigned.
- Home health experience preferred. Two to three years of physical therapy experience preferred (stated in client notes).
Preferred Qualifications
- Prior home health experience in evaluating home environments and coordinating interdisciplinary home care plans.
- Experience with patient education, caregiver training, and discharge planning.
Assignment Details
- Shift: Evenings, 5x8 schedule.
- Home visits required. Must provide reliable transportation and carry agency-required liability insurance.
