Minimum Qualifications
- Bachelor of Science in Physical Therapy from an APTA-approved program, or equivalent degree (Master's or Doctoral acceptable).
- Current State Physical Therapist license required prior to start of assignment.
- Current CPR certification required.
- Current drivers license, reliable transportation, and personal vehicle insurance required.
- 1-2 years of experience as a Physical Therapist in acute care or rehabilitation settings required.
- Experience with HCHB, OASIS, and home health documentation and workflows required.
- Ability to perform patient transfers and use durable medical equipment; ability to perform prolonged standing, walking, lifting, bending, pulling, and pushing.
- Ability to work a flexible schedule and travel locally within assigned market counties.
- Must read, write, and speak fluent English.
- Float to other local service areas as needed.
Preferred Qualifications
- Previous home health experience preferred.
- Experience ordering and adapting rehabilitative equipment preferred.
Assignment Details
- Shift Details: 5x8 schedule.
- Estimated weekly workload: approximately 2030 hours; expected productivity approximately 2030 units, with target productivity noted as 30 units.
- Primary service areas include Akron and Cleveland markets. Counties served include Cuyahoga, Summit, Stark, Lake, and Medina.
- On-call required when requested; on-call needs are subject to change based on staff and branch needs.
- Float required.
