Shift: 8:00 AM - 4:30 PM

Experience:

12 months of experience with 3 months worked in the last 12 months.

10 months of Home Health experience.

Experience with Epic is required.

Experience with Oasis is required.

Requirements:

Candidates must have a license (required for submission).

This role will require floating to multiple locations

Local & travel allowed. Candidates living Additional Details:

ER profiles must include the size of the facility / # beds and the Trauma Level designation (if any)

Required Skills/Experience:1 YR of Skilled Home Health Experience.

Required Credentials:Cannot have any hits/suspensions on license or any background hits from any point in the past.

Special Requests:RTO must be listed at time of submission.All positions at Providence Health & Services require that any and all lab items requested during onboarding be completed within 72 hours of signing a contract - no exceptions. This means candidates must be responsive and willing to go to a lab to complete health orders ASAP after signing. Failure to comply may lead to a pushed start or cancellation as assessed by the facility.References must cover at least 1 full year of relevant experience in specialty within the past 3 years.You must be able to complete labs within 48 hours of signing.Cannot be currently or recently (within 6 months) employed with network - must have left in good status, if former employee.First-time travelers not accepted.RTO Restrictions: Confirm with hiring manager

Unit Details:1:1 ratio. May visit up to 6 patients per day; 30 productivity points standard.Traveler is expected to have their own transportation.System groups clients to minimize drive and windshield time. Occasional maximum radius of 50+ miles.Parking Cost: Free

Shift & Scheduling:No weekends.No call.Orientation for all Oregon facilities is in Portland, OR for RN, LPN and CNAs.

5 days maximum time off

2 Manager/Supervisor references required from within the last 2 years
