Shift: 8:00 AM - 4:00 PM

Experience:

12 months of experience with 3 months worked in the last 12 months.

3 months of Epic experience is preferred.

Requirements:

Candidates must have a license (required for submission).

This role may require floating to additional units and locations

Travel only, local not allowed. Candidates must live >50 miles from facility to be submitted.

No current placement allowed at Program: RightSourcing Program - Atlantic Health.

Additional Details:

Required Skills/Experience:- Home health experience required.- Minimum of one (1) year of experience in acute, subacute or home care setting.

Required Credentials: Graduate of an accredited speech and language program with a minimum of a Masters degree.

Unit Details:- Home care role traveling to patient homes and facilities as indicated.- Performs baseline assessment of patient swallowing, communication, and cognitive abilities as indicated.- Instructs in communication and safe swallow, cognitive training as indicated.- Develops and instructs in a therapeutic exercise program.- Implements a program to restore function and achieve maximum functional status.- Provides discharge planning and referral to resources as needed.- Assumes responsibility for case management.- Use of personal vehicle is required for home visits.

Shift & Scheduling:- Mon-Fri.- No weekends.- No on-call.

7 days maximum time off
