Home Health Physical Therapist in Versailles, KY

Deliver essential care and support to patients in their own homes as a Home Health Physical Therapist. This contract opportunity focuses on helping individuals recover their mobility, strength, and independence with one-on-one therapy in a supportive environment. You will independently serve a diverse caseload, using a patient-centered approach and collaborating closely with an interdisciplinary care team.

Schedule:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Anticipated caseload of approximately 2532 points per week (approximately 57 patient visits daily)
• Field-based position with daily travel to patient homes (mileage reimbursement included)

Qualifications:

• Active and unencumbered Physical Therapist license in state of practice
• At least one year of recent home health physical therapy experience required
• OASIS experience necessary
• Familiarity with Home Care Home Base (HCHB) preferred
• Proficiency in functional and clinical assessments
• Strong communication, organization, and time management abilities
• Reliable transportation, current drivers license, and valid auto insurance
• Ability to work independently and navigate a flexible schedule

Duties:

• Complete comprehensive patient assessments, including OASIS when required
• Create and administer individualized treatment plans to improve functional outcomes
• Deliver skilled interventions (therapeutic exercise, neuromuscular re-education, gait and transfer training, fall prevention, and pain management)
• Provide patient and caregiver education on exercises, safety measures, and adaptive equipment
• Monitor patient progress, update care plans, and communicate with care team
• Maintain timely and accurate electronic documentation in HCHB platform
• Participate in interdisciplinary case conferences and continuous education sessions
• Ensure compliance with agency, Medicare, and regulatory standards
