Physical Therapist Home Health - Avondale, Colorado

A rewarding opportunity is now available for an experienced and compassionate physical therapist specializing in home health care. In this field-based position, youll deliver skilled therapy services directly in patients homes, supporting their journey toward improved mobility, independence, and quality of life. The role calls for clinical expertise, excellent organizational skills, and a patient-centered approach to care.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract

Key Responsibilities:

• Perform comprehensive physical therapy evaluations in patient homes, including OASIS assessments as required.
• Develop and adjust individualized treatment plans focusing on functional mobility, strength, gait, pain management, and safety.
• Administer interventions such as therapeutic exercises, neuromuscular re-education, gait and transfer training, and fall prevention strategies.
• Provide thorough patient and caregiver education on exercises, safety, and adaptive equipment.
• Maintain timely, accurate documentation using Home Care Home Base (HCHB).
• Collaborate with a multidisciplinary healthcare team to ensure coordinated, high-quality care.
• Participate in care conferences and ongoing professional development.
• Ensure compliance with agency policies and home health regulatory standards.

Qualifications:

• Active and unencumbered Physical Therapist (PT) license in the state of practice
• At least 1 year of recent home health physical therapy experience (field experience highly preferred)
• OASIS experience required; proficiency with Home Care Home Base (HCHB) preferred
• Strong functional assessment, time management, and communication skills
• Reliable transportation, valid drivers license, and current auto insurance
• Ability to work independently in a flexible field-based setting
