Home Health Physical Therapist in Lexington, KY

Experienced and compassionate physical therapists are needed to provide high-quality care to patients in their homes, ensuring improved mobility, strength, balance, and functional independence. This full-time contract assignment spans 13 weeks and offers the opportunity to develop meaningful relationships while making a direct difference in patients' lives.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Perform comprehensive physical therapy evaluations, including OASIS assessments as required.
• Develop and implement individualized treatment plans addressing functional mobility, strength, pain management, gait, safety, and balance.
• Deliver skilled interventions such as therapeutic exercise, neuromuscular re-education, gait training, transfer training, and fall prevention strategies.
• Monitor and document patient progress, adjusting care plans as needed and communicating updates to physicians and care team members.
• Educate patients and caregivers on exercise routines, safety precautions, use of assistive devices, and home exercise programs.
• Collaborate with nurses, occupational therapists, speech-language pathologists, medical social workers, and home health aides to provide coordinated care.
• Accurately complete documentation in the Home Care Home Base (HCHB) electronic system.
• Participate in interdisciplinary meetings, case conferences, and ongoing education.

Qualifications:

• Active Physical Therapist (PT) license in the state of practice, in good standing.
• At least 1 year of recent home health physical therapy experience preferred.
• Experience with OASIS and Home Care Home Base (HCHB) strongly desired.
• Reliable transportation, valid drivers license, and auto insurance.
• Ability to work independently and manage a field-based caseload.
• Strong communication, assessment, and organizational skills.
