Job Summary
The Occupational Therapist (OT) provides patient-centered occupational therapy services in Skilled Nursing, Long-Term Care, and Home Health settings. The OT evaluates patients, develops and oversees individualized plans of care, and supports patients in improving independence with activities of daily living (ADLs), functional mobility, and overall quality of life in accordance with regulatory and clinical best practices.
Key Responsibilities
Evaluate patients and develop individualized occupational therapy plans of care
Provide skilled occupational therapy treatments focused on ADLs, IADLs, cognition, strength, coordination, and functional mobility
Supervise and collaborate with Occupational Therapy Assistants (OTA/COTA) in accordance with state practice acts
Monitor patient progress, modify plans of care as appropriate, and communicate changes to the interdisciplinary team
Educate patients and caregivers on therapeutic techniques, safety strategies, adaptive equipment, and home modifications
Complete timely, accurate, and compliant documentation in the electronic medical record (EMR)
Ensure compliance with all federal, state, payer, and facility regulations, including HIPAA
Maintain a safe treatment environment and adhere to infection control and safety protocols
Demonstrate professionalism, compassion, and respect in all patient and family interactions
QualificationsQualifications
Graduate of an accredited Occupational Therapy program
Current Occupational Therapist license in good standing (state-specific)
Experience in skilled nursing, long-term care, or home health preferred (new graduates welcome to apply)
Strong clinical, communication, and time-management skills
Ability to work independently while contributing to a collaborative team environment
