Physical Therapist - Travel Contract

Looking for a change of scenery and a chance to help others. Join a traveling Physical Therapist team in Macomb, MI providing home health services. You will help patients recover from injuries and manage chronic conditions while working with a caseload that supports functional independence and safe discharge to community settings.

Minimum Requirements

  • Bachelors, Masters, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
  • Active Michigan Physical Therapy license required to start the assignment.
  • BLS certification from AHA or ARC may be required.
  • Experience with Home Health preferred; candidates new to home health will be considered.
  • Familiarity with Home Care Home Base EMR preferred.
  • Ability to meet productivity expectation of approximately 30 points per week.
  • Valid drivers license and reliable transportation. Mileage reimbursed at IRS standard mileage rate.

Preferred Qualifications

  • Prior home health caseload management experience.
  • Comfort performing home safety assessments and working independently in patient homes.
  • Experience with telehealth or remote documentation workflows.

Assignment Details

  • Shift days: M-F.
  • EMR: Home Care Home Base.
  • Interview format: Teams/Video Call.
  • Caseload geography: primary hub in Macomb with travel expected within roughly a one-hour radius depending on referral patterns.
  • On call: None.
