Peninsula Post Acute is seeking a compassionate, organized, and resident-focused Case Manager to join our team. This is a full-time position offering $25.00-$30.00 per hour, based on experience and qualifications, along with a comprehensive benefits package. Position Summary The Case Manager plays a vital role in ensuring residents receive exceptional, person-centered care through effective care planning, coordination, and advocacy. This position collaborates closely with residents, families, healthcare providers, insurers, and community partners to develop and implement individualized plans of care that promote optimal outcomes while responsibly managing resources and costs. Essential Responsibilities Collaborate with residents, families, physicians, therapists, nursing staff, and other healthcare professionals to develop, implement, and monitor individualized plans of care. Communicate regularly with residents and their families regarding care plans, therapy services (PT, OT, Speech Therapy), treatment progress, and available resources. Attend and actively participate in daily interdisciplinary meetings to facilitate communication, coordination, and continuity of care. Provide residents and families with education and support regarding diagnoses, treatment plans, care transitions, and disease progression. Monitor resident status and adjust care coordination activities based on changing clinical needs and circumstances. Prioritize and manage daily responsibilities by assessing the needs of new admissions, current residents, and discharging residents. Complete required documentation accurately and within established timeframes. Conduct utilization review activities to ensure residents receive appropriate, timely, and cost-effective services. Coordinate care and services among residents, healthcare providers, facility departments, financial services, and third-party payers. Manage admission, transfer, and discharge processes to support seamless transitions of care. Coordinate referrals to and from the facility with community resources, specialists, and post-acute care providers. Maintain accurate medical records, documentation, and regulatory compliance requirements. Exercise independent judgment and professional discretion when interacting with physicians, providers, residents, families, and insurance representatives. Foster positive working relationships and effective communication among residents, families, caregivers, physicians, nurses, therapists, and ancillary service providers. Demonstrate exceptional organizational and time-management skills in a fast-paced environment with competing priorities. Utilize electronic medical record systems and technology tools to support care coordination and documentation. Maintain professional, respectful, and ethical interactions with all residents, families, staff members, and community partners. Qualified non-licensed candidates with relevant healthcare experience will be considered. Previous experience in healthcare, case management, care coordination, discharge planning, social services, or a related field strongly preferred. Working knowledge of Medicare, Medi-Cal, Medicaid, and other healthcare reimbursement programs. Experience with PointClickCare (PCC) or other electronic medical record (EMR) systems preferred. Certified Case Manager (CCM) credential is a plus. Knowledge, Skills & Abilities Strong verbal and written communication skills with the ability to effectively educate and support residents, families, and interdisciplinary team members. Ability to interpret and apply policies, procedures, and regulatory requirements. Excellent organizational, critical thinking, problem-solving, and prioritization skills. Ability to manage multiple responsibilities and changing priorities while maintaining attention to detail. Proficiency with Microsoft Office applications and electronic healthcare systems.
