Contract Details:

• Company: Sunbelt Staffing, on behalf of a nationwide home health agency.
• Position: Full-time Home Health Physical Therapist (PT)
• Location: Tucson, AZ, and surrounding areas
• Schedule: Monday-Friday, 8:00 am - 5:00 pm, with one weekend per month on call
• Compensation: Negotiable hourly rate based on territory budget and candidate experience, blended pay packages available for travelers, weekly pay.
• Mileage: .48c visit to visit, 150-250 miles per week expected
• Benefits include medical, dental, vision, and 401k after 90 days.

Job Description:

• Conduct comprehensive assessments and develop personalized treatment plans tailored to each patient's needs.
• Provide skilled physical therapy services, including therapeutic exercises, manual therapy, and modalities, to enhance mobility, strength, and function.
• Collaborate with interdisciplinary team members, including physicians, nurses, and occupational therapists, to coordinate holistic patient care.
• Educate patients and caregivers on home exercise programs and safety measures to promote optimal recovery and independence.
• Document patient progress accurately and timely using electronic health record systems, including Homecare Homebase (HCHB).

Job Requirements:

• Active licensure as a Physical Therapist in Arizona.
• Active American Heart Association (AHA) or American Red Cross Basic Life Support (BLS) certification.
• Minimum of 1 year of experience in home health physical therapy.
• Proficiency with Homecare Homebase (HCHB) software, Oasis, and Outlook Point Care.
• Excellent communication and interpersonal skills.
• Ability to maintain productivity standards of 30 units per week.
