Position Summary

This position Clinical Case Manager Behavioral Health is with Aetnas Long-Term Services & Supports team and is a field-based position out of West or Central Chicago IL. The requirements is for candidates to hold an Illinois LCSW, LCPC, LMFT, and to travel 50-75% of the time to meet with members face to face.

Provide comprehensive healthcare management services to facilitate appropriate healthcare treatment, effectively manage healthcare costs and improve healthcare program/operational efficiency involving clinical issues Determines medical necessity/ appropriateness Facilitates optimal outcomes Identifies and follow through with continuous quality/ compliance opportunities. May also include identification of aberrance's and initiation of corrective action Educates/ empowers customers to ensure compliance, satisfaction and promote patient advocacy Optimize total costs Implementation and evaluation of policy based on usage and program directives Educate/empower colleagues at all levels to enable decision making at most appropriate level *

Required Qualifications

-Active IL BH clinical license - LCSW, LCPC, LMFT

-Minimum of 2 years of experience in behavioral health or social services field.

-Must reside in West or Central Chicago and Surrounding Areas, applicable zip codes 60651, 60644, 60624. 60612, 60623, 60622, 60608, 60632, 60616, 60647, or 60639

- Must have a valid IL Drivers License

- Willing and able to travel up to 50-75% of their time to meet with members face to face in West or Central Chicago and Surrounding Areas

- Reliable transportation required; Mileage is reimbursed per our company expense reimbursement policy

Preferred Qualifications

- Case management and discharge planning experience

- Managed Care experience

- Microsoft Office experience

- Experience working with HIV and AIDS population

Education

Master's degree in social services, human services or behavioral health required

Pay Range

The typical pay range for this role is:

$66,575.00 - $142,576.00

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
