Park Nicollet is looking to hire a Physical Therapist Assistant to join our Home Care team! Position Summary: The Physical Therapist Assistant is responsible for assuring that the PNHS values of Head + Heart, Together, are achieved through daily departmental operations. This includes, but is not limited to, direct patient care, professional growth and development, compliance with regulatory standards and participation in process improvements to advance patient care outcomes and satisfaction. The Physical Therapist Assistant will provide patient therapy treatment and teaching to patients and families in their home in accordance with the plan of care established by the Physical Therapist. The Physical Therapist Assistant will communicate in a timely, clear, and professional manner with staff, patients, patient family members, physician, and other internal/ external customers. The staff member will demonstrate skills in listening, conflict management, system problem solving, decision making and process improvement. Physical Therapist Assistants will practice in compliance with professional standards of practice and professional code of ethics. The assistant supports the following community care processes: admission, discharge, referrals to other disciplines, billing, and care delivery. This position will help in the Central/West territory (typically Minnetonka, Hopkins, St. Louis Park areas). Work Schedule: M-F 8am - 4:30pm. 4 days/week. Occasional weekends. Required Qualifications: Education, Experience or Equivalent Combination: Physical Therapy Assistant Diploma from an accredited school. Licensure/ Registration/ Certification: Current license with Minnesota Board of Physical Therapy Knowledge, Skills, and Abilities: Must be able to function independently with minimal supervision. Demonstrates knowledge of age-related growth and development principles to provide appropriate service regarding primary diagnosis and multiple co morbidities affecting care. Demonstrate initiative, judgement, critical thinking, and ability to problem solve independently under the direction of a Physical Therapist. Proficiency with communication and data processing equipment, clinical apparatus related to patient care in homecare, electronic medical record system. Preferred Qualifications: Work experience as a Physical Therapist Assistant for at least 3 years preferred. Experience in homecare desirable.
