What We’re Looking For
· Licensed Physical Therapist in New Jersey
· Degree from an accredited Physical Therapy program (DPT, MSPT, or BSPT)
· CPR certification (preferred)
· Experience in home care, Part A or B, or outpatient.
· Reliable transportation
· A compassionate, patient-centered mindset and strong communication skills
· Familiarity with electronic health record systems (e.g., WebPT or Hello Note)
· New Graduates are welcome to apply
Key Responsibilities:
· Evaluate and treat patients with physical disabilities, injuries, or functional limitations
· Develop and implement individualized physical therapy care plans
· Provide therapeutic exercises, manual therapy, gait training, and neuromuscular re-education
· Deliver orthopedic, post-surgical, and neurological rehabilitation services
· Educate patients and caregivers on home exercise programs and safe mobility practices
· Maintain documentation in electronic medical records in accordance with Medicare and CMS regulations
· Assist with fall prevention, balance training, and pain management strategies
