We are seeking a licensed and experienced Physical Therapist (PT) to join our Home Health Rehabilitation Team. This role provides in-home physical therapy services to adult patients with orthopedic, neurological, and work-related conditions. The ideal candidate will be skilled in patient evaluation, treatment planning, and therapeutic intervention, and will collaborate with a multidisciplinary team to deliver high-quality, compassionate care.
Responsibilities
Conduct comprehensive physical therapy evaluations in patients' homes.
Develop and implement individualized treatment plans to improve mobility, strength, and functional independence.
Educate patients and caregivers on therapeutic exercises, home safety, and recovery strategies.
Monitor patient progress and adjust treatment plans as needed.
Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated, interdisciplinary care.
Supervise and provide clinical direction to Physical Therapy Assistants (PTAs).
Maintain accurate and timely documentation in compliance with regulatory and EMR standards.
Qualifications
Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program.
Current North Carolina Physical Therapy License.
CPR Certification required.
Minimum of 1 year of clinical experience in physical therapy.
Valid North Carolina driver's license with a clean driving record.
Preferred Qualifications:
Experience in home health, outpatient rehab, or community-based therapy.
Strong communication, clinical reasoning, and interpersonal skills.
Ability to work independently and manage a flexible, autonomous schedule.
Familiarity with Medicare guidelines, OASIS documentation, and electronic medical records (EMR) is a plus.
