Description
  • Assess patients in both inpatient and outpatient environments and develop personalized treatment plans that cater to their unique needs.
  • Implement therapeutic interventions that focus on enhancing mobility, strength, balance, and overall function.
  • Work in partnership with nurses, physicians, case managers, and other rehabilitation professionals to ensure optimum patient outcomes.
  • Guide patients and their families through understanding therapeutic objectives, discharge processes, and home exercise routines.
  • Maintain thorough documentation of evaluations, treatments, and progress while adhering to facility policies and regulatory standards.
  • Engage in care team rounds and interdisciplinary meetings to coordinate care effectively.
Education
  • Doctorate in Physical Therapy (DPT) or a comparable degree from a recognized institution.
Certifications
  • Active state licensure as a Physical Therapist
  • CPR certification desired
  • Additional specialized certifications (e.g., neurological, geriatric) are advantageous but not mandatory
Skills
  • Prior experience preferred but not strictly required.
