- Assist patients with physical therapy exercises and mobility techniques to improve their independence.
- Communicate effectively with patients and team members to ensure clear understanding of care plans.
- Maintain accurate patient records and ensure compliance with HIPAA and state regulations.
- Adapt care plans to meet the unique needs of each patient's home environment.
- Utilize EMR systems and mobile health apps for scheduling and reporting.
- Provide empathetic and culturally sensitive care to diverse patient populations.
- Ensure safe patient transfers and provide mobility support as needed.
- Associate's degree in Physical Therapy Assisting or related field.
- Physical Therapy Assistant Certification and PTA License.
- Strong knowledge of patient care techniques and medical terminology.
- Excellent communication skills for effective patient and team interaction.
- Ability to work independently and collaboratively in a team.
- Proficiency in using EMR systems and mobile health apps.
- Home care experience and familiarity with in-home patient care protocols.
- CPR/First Aid Certification (preferred or required depending on state regulations).
- Drivers license and reliable transportation for traveling between patient homes.
- Empathy, patience, and cultural sensitivity.
- Problem-solving skills to adapt care plans to patient needs.
15 PTO days annually after 90 days, usable for vacation or sick time.
Accrual: +1 day per year of service, up to 20 days max.
8 paid holidays: New Years Day, MLK Day, Memorial Day, Juneteenth, July 4th, Labor Day, Thanksgiving, Christmas.
