Duties include, not limited to:

Screens MHRRTP admissions face to face or via VVC/telephone.

After compiling pertinent clinical informion, evalues consults with interdisciplinary trement team for approprieness for residential level of care.

Attends daily team rounds to assess and plan Veteran care and disposition (transition of care) needs.

Performs Mental Stus Exams (MSEs), Uniform Biopsychosocial Assessments, Uniform Biopsychosocial Continuing Assessments, and other diagnosis-specific assessments (e.g., CAPS-5 for PTSD) and Measurement Based Care instruments (e.g., BAM, PHQ9, PCL- 2 5) as approprie within scope of SW practice to assess individualized Veteran care needs.

Facilite completion of advanced directives and psychiric advance directives with Veterans and their families.

Develops and maintains/revises trement plans, in collaborion with the MHRRTP interdisciplinary trement team and in accordance with local policy and accrediting body standards, on all assigned pients.

Performs advanced level case management interventions used in the trement of Veterans with poly-trauma injuries, spinal cord injuries, traumic brain injuries, visual impairment, post-traumic stress disorder, addictions, and serious mental illness.

Provides suicide screening and comprehensive suicide risk evaluions, develops safety plans and delivers lethal means safety counseling, and completes SBOR and reled documention/reporting regarding suicide/overdose risk management.

Works in collaborion with the Suicide Prevention Team and the Local Recovery Coordinor and is well versed in SOPs for suicide prevention and clinical management of pients high risk for suicide or other adverse outcomes (e.g., those identified by REACH VET).

Provides (VA/DoD CLINICAL PRACTICE GUIDELINE FOR MANAGEMENT OF MENTAL HEALTH and SUBSTANCE USE DISORDERS) Evidence Based practices/interventions to include individual, group, couples and family therapy, and other EBPs with in scope of SW practice.

Develops discharge plan in concert with trement team, Veteran, and family (as approprie).

Coordines transitional care for Veterans needing referral to the outpient mental health clinic as well as admission to the inpient psychiry unit and/or other VA services.

Coordines psychosocial needs of the Veteran to include temporary housing, long term care, transportion and post-discharge MH, Medical, and SUD care services and associed transportion.

Provides discharge follow-up phone calls to veterans who are discharged regularly or administrively for trement engagement and may provide listed services to 1W Veterans (inpient MH), ED and or medical floors

Ensures Same Day Substance Use and Mental Health Access either Face to Face or via Phone, consistent with MHS SOP.

Must have 1 year of experience providing evidence based psychotherapy for substance use disorders (SUD; CBT-SUD, MI/MET, CM).

Must have 1 year of experience providing evidence based psychotherapy for posttraumic stress disorder (PTSD; CPT, PE, EMDR).

Must have current valid driver's license for the position as driving for community therapeutic outings is a shared responsibility for psychotherapist staff.

Preferred least 1 year of experience in inpient or residential mental health/substance use recovery program.

Work Schedule : Monday - Friday 8:00 a.m. to 4:30 p.m. work schedule, but work schedule may be adjusted to meet the needs of the service.

Basic Requirements :

United Stes Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.

English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct pient care positions unless they are proficient in basic written and spoken English.

Master's degree in Social Work from a school of social work fully accredited by the Council on Social Work Educion (CSWE). Gradues of schools of social work th are in candidacy stus do not meet this requirement until the school of social work is fully accredited. Note: A doctoral degree in Social Work may NOT be substituted for the master's degree in Social Work. Verificion of the degree can be made by going to to verify th the social work degree meets the accredition standards for a masters of social work.

Licensed or certified by a ste to independently practice social work the advanced practice level which included an advanced generalist or clinical examinion, unless they are grandfhered by the ste in which they are licensed to practice the advanced practice level (except for licenses issued in California, which administers its own clinical examinion for advanced practice) and they must be able to provide supervision for licensure.

Grade Determinions: In addition to the basic requirements sted above, the following qualificion criteria must be met for each grade. The candide's qualificions must clearly demonstre the level of competence required for the grade.

Experience: At least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Senior social workers have experience th demonstres possession of advanced practice skills and judgment and are experts in their specialized area of practice. Senior social workers may have certificion or other post-masters training from a nionally recognized professional organizion or university th includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty.

Demonstred Knowledge, Skills, and Abilities . In addition to the experience above, candides must demonstre all of the following KSAs:

Skill in a range of specialized interventions and trement modalities used in specialty trement programs or with special pient populions. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management.

Ability to incorpore complex multiple causion in differential diagnosis and trement within approved clinical privileges or scope of practice.

Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluions to improve trement services and to design system changes.

Ability to provide specialized consultion to colleagues and students on the psychosocial trement of pients in the service delivery area, as well as role modeling effective social work practice skills.

Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area.
