Description
  • Conduct thorough evaluations of patients in both inpatient and outpatient environments and create personalized treatment plans accordingly
  • Administer targeted physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function
  • Engage in collaborative efforts with nurses, physicians, case managers, and other rehabilitation professionals to optimize patient outcomes
  • Educate patients and their families on therapeutic objectives, discharge strategies, and home exercise routines
  • Document evaluations, treatment sessions, and patient progress in line with facility policies and regulatory standards
  • Participate in care team rounds and interdisciplinary meetings to ensure seamless coordination of care
Education
  • Doctor of Physical Therapy (DPT) or a relevant advanced degree from an accredited program
Certifications
  • Valid state licensure as a Physical Therapist
  • CPR certification is highly recommended
  • Additional specialized certifications (e.g., neurological, geriatric) will be viewed favorably, but are not mandatory
