- Conduct comprehensive functional assessments (ADLs, IADLs, cognition, psychosocial skills) pre- and post transition.
- Develop and implement individualized treatment plans that aligns client-centered goals and values with transitioning into lower level of care.
- Implement evidence-based OT interventions (individual and/or group) utilizing an occupation based, strengths based approach
- Collaborate with interdisciplinary teams (e.g. care/case managers, SNF/SMHRF facilities staff, RN staff, housing specialists) to ensure conti nuity of care.
- Provide in-services or consultation to other team members regarding functional assessment, adaptive strategies, and environmental modifications.
- Attend meetings to consult with teams regarding client progress, behavior, and response to treatment.
- Instructs clients as needed in the use of adaptive equipment
- Address environmental modifications and recommend/provide adaptive equipment as needed for community/independent living
- Conduct interviews with family members, guardians, and/or significant others, as appropriate about the clients past and present functional levels, capabilities and performances.
- Provide community-based and in-home occupational therapy to facilitate skill-building for independent living (i.e., cooking, budgeting, transportation use)
- Promote self-efficacy, community participation, and social connection through structured activities and coaching
- Maintain accurate and timely documentation, such as completing assessments, written and verbal recommendations, treatment plans, progress notes, and discharge summaries, in accordance with agency policy, Medicaid/Medicare, and regulatory standards.
- Plans and/or assists with client discharge and provides appropriate recommendations for selection/use of adaptative equipment, compensatory strategies and support programs as needed/identified
- Provide caregiver and client education on strengths and needs that support client success in community.
- Review medical, clinical charts, and/or other pertinent documents for the Class Member made available by the nursing facility and documentation from Care Management team as appropriate.
- Receive input from Supervisor and provide feedback regarding administrative concerns, clinical consultations and caseloads
- Interpret and communicate agency and program policies and procedures
- Participate in orientations and training opportunities as offered by the program and required for credentials/by the state
- Provide direct supervision to Level II Occupational Therapy Students as assigned
- A majority of the employees time will be spent in the community or nursing facility settings, driving to and from Nursing Facilities, and providing care in residences or community areas
- While performing the duties of this job, the employee will have access to office space but will have ability to work remotely
- In supporting clients to live independently, employee may be exposed to varying home conditions
- The employee is required to travel by automobile to and from KYC office locations on a weekly basis, as well as frequent client visits in nursing homes, client residencies and nursing home facilities in the Chicagoland area, and occasional meetings/trainings in and around the Chicagoland area.
- Masters Degree
- Occupational Therapist with current Illinois license
- One-year occupational therapy experience in nursing home setting or community based preferred
- Preferred experience with clients that experience mental health and complex medical conditions
- Current and valid drivers license required.
- Daily access to a personal vehicle is required.
- Full Time, Exempt, 37.5 hour work week
