Contract Details:

  • Position: Full-time Home Health Physical Therapist (PT)
  • Type: Full-Time Contract, 13 weeks, with possibility of extension
  • Location: Agawam, MA??????, and surrounding areas
  • Hours: Full-Time, Scheduled 40 hours per week - no guaranteed hours
  • Schedule: Monday-Friday, 8:00 am - 5:00 pm
  • Productivity: 25-32 per week, up to 35 patients per week
  • Compensation: Negotiable hourly rate based on territory budget and candidate experience, with mileage reimbursement.
  • Benefits: Medical, Dental, Vision, 401 (k) after 90 days of employment.

Job Requirements:

  • Active licensure as a Physical Therapist in Massachusetts.
  • Active American Heart Association (AHA) or American Red Cross Basic Life Support (BLS) certification.
  • Minimum of 1 year of experience in home health physical therapy.
  • Proficiency with Homecare Homebase (HCHB) software, Oasis, and Outlook Point Care.
  • Excellent communication and interpersonal skills.
  • Ability to maintain productivity standards of 30 units per week.

Job Responsibilities:

  • Conduct thorough assessments of patients' physical conditions and functional limitations within their homes.
  • Develop individualized treatment plans tailored to each patient's needs and goals.
  • Provide skilled physical therapy interventions, including therapeutic exercises, manual therapy techniques, and gait training.
  • Collaborate closely with interdisciplinary team members to ensure coordinated and holistic patient care.
  • Educate patients and caregivers on injury prevention, proper body mechanics, and adaptive equipment use.
  • Maintain accurate and timely documentation of patient assessments, interventions, and progress using electronic health record systems.

Important Note: Due to this being a contract opportunity with limited training, one year of experience and proficiency in Homecare Homebase are required.
