As a Physical Therapist in our facility, your responsibilities will include:

- Evaluating patients in either inpatient or outpatient settings and crafting personalized treatment strategies

- Administering interventions centered on enhancing mobility, strength, balance, and overall function

- Collaborating closely with a multidisciplinary team, including nurses, physicians, and case managers to optimize patient outcomes

- Educating patients and their families on therapeutic objectives, discharge planning, and home exercises

- Maintaining accurate documentation of evaluations, treatments, and patient progress in line with both facility protocols and regulatory guidelines

- Participating in care team meetings and interdisciplinary conferences to ensure comprehensive and coordinated care

To thrive in this role, you'll need:

- A Doctorate in Physical Therapy (DPT) or an equivalent qualification from an accredited institution

- State licensure as a Physical Therapist; CPR certification preferred

- Prior experience strongly preferred, particularly in treating patients with complex medical conditions
