Travel Physical Therapist Home Health contract job - Colchester, VT

Sunbelt Staffing is seeking an experienced and compassionate Physical Therapist to join a dedicated home health team in Colchester, Vermont. This assignment offers the opportunity to provide individualized care while enjoying the natural beauty and charm of northern Vermont.

• Contract Length: 16 weeks

• Shift: 8-hour day shifts

• Setting: Home Health

• Perform comprehensive assessments and create individualized treatment plans for home health patients.

• Deliver skilled therapy services including gait training, balance retraining, strengthening, and mobility exercises.

• Provide patient and caregiver education for safe home exercise programs and fall prevention.

• Collaborate with the interdisciplinary team to ensure coordinated, patient-centered care.

• Document all interventions and progress in compliance with regulatory and facility standards.

• Active Vermont Physical Therapist license.

• Minimum 1 year of recent home health PT experience preferred.

• Proficiency with electronic documentation systems (Homecare Homebase preferred).

• Current BLS certification.

• Strong clinical judgment and the ability to work independently.
