Essential Functions:

  • Provide and coordinate a continuum of treatment services, including collaborating with Care Managers, Transition staff, RNs and others. Operate as part of a treatment team.
  • Carry a caseload of Class Members, providing a spectrum of occupational therapy services as assigned by Supervisor.
  • Meets reporting, productivity, and service deliverable requirements identified by LSSI, DHS, and funding bodies.
  • This role requires the Occupational Therapist to work as part of a multi-disciplinary team.
  • Collaborates with other LSSI programs/departments, including ACT and CST, as needed.
  • Prepares and maintains records and reports related to patient evaluation and treatment plans.
  • Provide skill that address the full scope of patient's needs.
  • Complete EHR documentation in a timely manner and in compliance with DHS and Rule 140 guidelines.
  • Ensure adherence to ethical standards of treatment, including NASW Code of Ethics.
  • Familiarity with Emergency Procedures.
  • Demonstrate professional, positive behavior and carry out responsibilities with integrity, treating clients, families, and other LSSI workforce members, and collaborative organizations and/or individual in a dignified, respectful, honest, and fair manner.

Position Qualifications:

  • Master's degree in occupational therapy required.
  • 1-3 years of experience in mental health field is preferred.
  • OT/L (Occupational Therapist, License) is required.
  • Proficiency in EHR documentation and review.
  • Familiarity with accreditation and licensure standards.
  • Excellent communication, organization, presentation and pc/computer skills (including proficiency with Microsoft Office - Outlook, Word, Excel and PowerPoint) along with other related software.
  • Excellent written and verbal communication skills.
  • Must have an Illinois license.
  • Regular driving is required.
  • Access to reliable transportation.
  • Valid Driver's License in good standing for the state of residency and insurance.
  • Valid Illinois statutory and liability insurance coverage is required.
