Essential Functions:
- Provide and coordinate a continuum of treatment services, including collaborating with Care Managers, Transition staff, RNs and others. Operate as part of a treatment team.
- Carry a caseload of Class Members, providing a spectrum of occupational therapy services as assigned by Supervisor.
- Meets reporting, productivity, and service deliverable requirements identified by LSSI, DHS, and funding bodies.
- This role requires the Occupational Therapist to work as part of a multi-disciplinary team.
- Collaborates with other LSSI programs/departments, including ACT and CST, as needed.
- Prepares and maintains records and reports related to patient evaluation and treatment plans.
- Provide skill that address the full scope of patient's needs.
- Complete EHR documentation in a timely manner and in compliance with DHS and Rule 140 guidelines.
- Ensure adherence to ethical standards of treatment, including NASW Code of Ethics.
- Familiarity with Emergency Procedures.
- Demonstrate professional, positive behavior and carry out responsibilities with integrity, treating clients, families, and other LSSI workforce members, and collaborative organizations and/or individual in a dignified, respectful, honest, and fair manner.
Position Qualifications:
- Master's degree in occupational therapy required.
- 1-3 years of experience in mental health field is preferred.
- OT/L (Occupational Therapist, License) is required.
- Proficiency in EHR documentation and review.
- Familiarity with accreditation and licensure standards.
- Excellent communication, organization, presentation and pc/computer skills (including proficiency with Microsoft Office - Outlook, Word, Excel and PowerPoint) along with other related software.
- Excellent written and verbal communication skills.
- Must have an Illinois license.
- Regular driving is required.
- Access to reliable transportation.
- Valid Driver's License in good standing for the state of residency and insurance.
- Valid Illinois statutory and liability insurance coverage is required.
