We are seeking a Licensed Physical Therapist (PT) to join our Therapy Department. In this role, you'll provide comprehensive physical therapy services including evaluations, treatment planning, ongoing reassessments, and discharge planning while collaborating with residents, families, and healthcare professionals. You'll develop individualized care programs, modify treatments as resident needs change, and ensure optimal functional outcomes. You hold a current Ohio Physical Therapy license (Bachelor's degree required; Master's preferred).

Responsibilities
Administers physical therapy in compliance with Federal, State, local, and Community First Solution's standards, policies, and guidelines ensuring quality care and service to Community First Solution's residents.
Interacts with health professionals, residents, families, and support groups to ensure the maximum rehabilitation, while serving as a client advocate at all times.
Evaluates residents by administering muscle, neurological, gait, and functional tests.
Establishes treatment goals based on the evaluation of the resident in conjunction with the resident's needs.
Develops treatment programs and provides ongoing assessments and modification as changes in resident's condition occur.
Prepares, transmits, and maintains timely and accurate records to include the initial evaluation, progress notes, discharge summary, and attendance records.
Complies with OSHA standards and ensures the safety of the resident in their care at all times.
Adheres to infection control policies, professional therapeutic services policies and all Community First Solution's policies and procedures.
Adhere to all policies and procedures of the organization and all federal and state laws regarding the privacy of protected health information.
Adhere to all policies and procedures of the organization regarding the use of technology and technology-based information.
Responsible for welcoming and on-boarding new employees.
Qualifications
Education: Bachelor's degree from an accredited college in Physical Therapy (Master's degree preferred).
Licenses or other required certifications: Current Ohio Physical Therapy License.
Experience: At least two years physical therapy experience in long-term health care setting.
