JOB SUMMARY:
As a Home Health Physical Therapist, you'll empower patients to reach their highest level of function and independence-right in the comfort and familiarity of their own homes. Drawing on your expertise in physical assessment, therapeutic intervention, and patient education, you'll help individuals regain mobility, restore confidence, and enhance their overall quality of life.
In this role, you'll collaborate closely with physicians and the interdisciplinary home health team to develop and implement individualized care plans that support recovery, reduce the risk of hospital readmission, and promote long-term wellness. You'll be responsible for evaluating, planning, directing, and administering physical therapy services as prescribed by a licensed physician, nurse practitioner, or physician assistant.
You will guide patients in reaching their maximum potential through tailored treatment strategies, while coordinating, delegating, and supervising tasks assigned to supportive staff to ensure the highest standard of care.

Regulatory Requirements:
v Current state Physical Therapy licensure, graduate of an approved school of Physical Therapy or graduate of accredited college or university with a certificate in Physical Therapy.
v Previous experience in all aspects of physical therapy preferred.
v Current BCLS certification.
Language Skills:
v Ability to read and communicate effectively in English.
v Strong written and verbal skills.
Skills:
v Basic computer knowledge.
v Knowledge of physical medicine, physical therapy modalities, anatomy and physiology.
v Knowledge of physical therapy machine operation, maintenance and repairs of same.
