• Assess patients in both inpatient and outpatient environments and create tailored therapeutic plans
• Administer physical therapy interventions focusing on enhancing mobility, strength, balance, and overall function
• Collaborate with a dedicated team of nurses, physicians, case managers, and rehab specialists to achieve optimal recovery outcomes
• Educate patients and their families about therapeutic goals, discharge planning, and home exercise programs
• Ensure comprehensive documentation of evaluations, treatments, and progress in alignment with facility protocols and regulations
• Participate in interdisciplinary team rounds and conferences to foster coordinated care

• Doctorate in Physical Therapy (DPT) or a comparable degree from a recognized program

• State licensure as a Physical Therapist
• CPR certification preferred
• Specialized certifications (neuro, geriatric) are advantageous but not mandatory

• Prior clinical experience strongly preferred
