SLP (Speech Language Pathologist), Home Health - part-time
Nightingale Home Healthcare was established 26 years ago with one purpose - to put our patients first. Nightingale provides patients with around the clock care and support. We offer a competitive salary, flexible scheduling, mileage reimbursement, 3 weeks of PDO per year, one-on-one patient care, referral bonuses up to $500, and regular evaluations with annual merit increases.
We are currently looking SLP part-time/per diem position to be able to provide the service in certain areas: Greenwich, Stamford, Norwalk, Westport.
The SLP interprets and evaluates recommendations and orders. Assigned patient's capabilities and disabilities, plans and carries out a progressive treatment program, and coordinates treatment program with total rehabilitation program.
Duties Include:
Effective patient care using approved treatment techniques and procedures. Evaluation of patient's condition, functioning level and treatment goals and keeping current records.
After evaluating the patient, the therapist is responsible for planning administering and supervising the STA in accordance with the treatment plan.
Administers appropriate evaluations to assess patient's level of function and develops therapy goals. Develops treatment plan for each patient and performs appropriate individual treatment of assigned patients. Documents patient's treatment in medical chart daily.
Will provide therapy services as outlined in the Plan of Care.
Assists patient and family with development and review of home program, evaluation and review of adaptive equipment needed in home setting and evaluation of onsite home setting when necessary.
Charts services the Therapist has rendered to the patient, notes changes in the patient' condition and/or family and home situation, makes revisions in the care plan as needed, records supervisory visits, evaluates patient care and progress and closes charts of discharged patients by preparing Discharge Summaries and obtaining Discharge orders.
Maintains confidentiality of client's situation, records and services provided according to HIPPA regulations
Evaluates the effectiveness of his/her therapy service to the individual and family.
Submits all patient notes and/or visit records on a timely basis.
Obtains orders for paramedical services and submits referral to the Director of Clinical Services, i.e., nursing services, medical social services, etc.
Coordinates total patient care.
Qualifications
Qualifications Include:
Licensed SLP in CT.
Minimum of 1 year as an SLP experience preferred.
Physical within 6 months prior to first patient contact.
Current TB results (within last 12 months) and CPR Certification.
Valid driver's license and proof of liability insurance on automobile.
Ability to attend 1-day orientation at our Stamford office.
