Willow Creek Healthcare Center is seeking a skilled and compassionate Physical Therapist Assistant (PTA) to join our rehabilitation team. We are a skilled nursing facility committed to providing excellent patient-centered care and fostering a supportive work environment for our staff. Key Responsibilities: Deliver physical therapy treatments as directed by the supervising Physical Therapist Accurately record daily treatment notes and weekly progress reports per PT Board standards Assist in department upkeep and equipment maintenance Participate in care planning and interdisciplinary rehab conferences Collaborate with nursing staff to train Restorative Aides Communicate effectively with healthcare team members regarding patient care plans and progress Conduct in-service training for facility staff as needed Educate patients, families, and caregivers in preparation for discharge Ensure documentation meets all facility and regulatory requirements Qualifications: Licensed as a Physical Therapist Assistant in the state of California Experience with Casamba and PointClickCare preferred Strong written and verbal communication skills Knowledge of therapy best practices and documentation standards Must maintain continuing education per state board guidelines Ability to lift/push/pull up to 100 lbs occasionally and meet physical demands of the role Ability to relate positively and professionally with patients, families, and team members Why Join Willow Creek?
