Responsibilities:
Deliver physical therapy treatments according to physician orders and the state Physical Therapy Practice Act.
Evaluate patients' therapeutic needs and develop personalized care plans, adjusting as needed.
Educate patients and caregivers on the use and care of therapeutic equipment.
Report patient progress and any changes in condition to the supervising physician.
Participate in discharge planning and submit clinical progress reports.
Supervise physical therapy assistants and collaborate with other healthcare team members.
Qualifications
Degree in Physical Therapy from an accredited program (Bachelor's or Master's).
Licensed to practice as a Physical Therapist in the state.
Minimum of two years of experience in physical therapy; community/home health experience preferred.
Strong communication, organizational, and problem-solving skills.
CPR certification and a valid driver's license with reliable transportation.
