ESSENTIAL DUTIES AND RESPONSIBILITIES:

1. Provides therapy and performs selected physical therapy interventions under the direction and supervision of the Physical Therapist. Responsible for service and operational excellence of all assigned activities to ensure the delivery of quality services and/or outcomes required to meet or exceed the expectations of those utilizing or impacted by the department.

2. Obtains data related to the interventions provided. Makes modifications in selected interventions either to progress the resident as directed by the physical therapist or to ensure resident safety and comfort. Demonstrates knowledge and appropriate application of precautions and contraindications to physical therapy treatment procedures and modalities. Carries out the d/c plan as determined by the PT

3. Maintains proper documentation This includes but is not limited to: creating and maintaining a resident record for each patient treated; completing documentation per policy including timeliness and medical necessity support; assures therapy billing and labor entries are complete, accurate and timely.

4. Communicates effectively by maintaining communication with supervisory PT; participating in supervisory visits with the supervising PT as required by the State Practice Act; requesting re examinations from the supervising PT when a change in resident status is identified; partnering with multi-disciplinary team to carry out plan of care and advocate for resident and communicating with the interdisciplinary team regarding treatments and discharge planning. Relates professionally, positively, and cooperatively with residents, family members, and other employees at all levels.

5. Participates in quality and process improvement activities, as directed.

6. Participates in continuous survey readiness, as directed.

7. Maintains and applies current skills and knowledge through continuing education and in-service programs. Maintain knowledge of Medicare Policies and Regulations, LCDs, and NCDs that directly impact care. Participates in in-service training and completes all mandatory training within designated time frame 8. Maintains a clean, safe and professional department. Presents with a professional appearance and attitude at all times. Adheres to facility dress code if applicable. Maintains regular attendance.

9. Participates in census building and marketing activities, as directed

10. Participates in clinical program development and implementation, as directed.

11. Carries out responsibilities in compliance with federal, state, local laws and regulations, and with facility philosophy, policies and procedures

EDUCATION and/or EXPERIENCE: A graduate of an approved education course for physical therapy assistants. Prior long-term care and EMR experience strongly preferred.

CERTIFICATES, LICENSES, REGISTRATIONS: Driver's license per position requirements. Licensed, if applicable, in the state in which they will be practicing.
