The Case Manager for our Outpatient Program is responsible for clinical and medical case management for our ABH clients.

Duties & Responsibilities

  • Obtain initial insurance authorizations.
  • Obtains extensions for insurance authorizations.
  • Check daily attendance and maintain admission log and distribute weekly census report.
  • Facilitate groups.
  • Document all collateral contacts and services in our EMR system.
  • Track and collect fees, when applicable.
  • Meet with clients to develop and review treatment plans.
  • Link clients to appropriate services, such as primary care, transportation, and psychiatric.
  • When discharge is planned, arrange for aftercare appointments.
  • Phone or letter outreach for clients who drop out.
  • Participate in team meetings and program supervision.
  • Markets the program.
  • Participate in ongoing development of curriculum for ABH.
  • Provide one-on-one counseling.
  • Complete discharge summaries/ Disenrollment MIS (EIM/ESM).
  • Answer phone inquiries about the program from prospective client and referral sources.
  • All other duties as requested.
Experience and Skills:

Duties & Responsibilities

  • Bachelor's degree, preferred.
  • CAC/CADAC, preferred.
  • Demonstrated effectiveness in working with and engaging an addicted population.
  • A minimum of two years experience in substance dependence and/or mental health field.
  • Strong organizational skills.
  • Ability to function independently with minimal supervision.
  • Solid understanding of insurance procedures and payor system.
  • Knowledge of community services - especially SSTAR services.
