Job Description
Position Overview: A License Occupational Therapist with a minimum of 2 years of clinical experience in an out-patient or ambulatory care setting. With ability to perform assigned task independently and/or with minimal supervision. Responsible for evaluating occupational therapy needs and implementing comprehensive and individualized treatment to assigned patients. Supervises Occupational Therapy Assistant as assigned by the director.
Responsibilities:
Perform evaluation and treatment. Utilizing evaluation/re-evaluation findings, chooses appropriate equipment, therapeutic techniques and/or modalities to achieve optimum individualized patient results.
Prepare daily work schedule allocating adequate time for completion of assignments.
Provide treatment that include but is not limited to therapeutic exercises, transfers, activities of daily living, pain management, balance training, wheelchair mobility and management with documentation in progress note.
Review patient's medical chart prior to initial contact and subsequently at periodic intervals to obtain pertinent information and consult with Physician and/or appropriate members of the interdisciplinary team.
Perform patient evaluation/re-evaluation which may include goniometry, manual muscle testing, transfers, ADL, skin condition, sensory testing, standardize testing and document appropriately.
Interpret evaluations/re-evaluation findings and formulates individual treatment programs with time frames for short-term and long-term goals and record progress on EMR.
Establish treatment programs for individual patients.
Maintain proper and timely records concerning patient treatment, attendance and progress as well as short and long-term goals.
Assign treatment procedures and supervise progress of patients treated by OT Assistants.
Utilize Rehabilitation Technicians to aid in administering appropriate patient treatment as per their direction.
Write evaluations and maintain progress notes and records in the EMR system.
Check and prepare treatment areas and equipment to insure patient safety and reports to Supervisor accordingly.
Provides patient and/family education of safety precautions and Home Exercise Program as appropriate.
Participate in facility in-service and education.
Maintain accountability with respect to safety, efficiency, effectiveness, ethics and legal issues.
Supervise and instruct Occupational Therapy students, Rehabilitation Technicians, and Volunteers as required.
Assumes other responsibilities as agreed upon by the Occupational Therapy Supervisor/Rehabilitation Director
Minimum Qualifications
Possess 4-5 years of formal education and Bachelor/Master's degree in an accredited Occupational Therapy program, holds an active NYS License and NBCOT
Possess at least 2 years of clinical experience in an Out-patient and/or ambulatory care setting. Additional clinical experience in an acute, sub-acute and/or SNF setting is preferred
Ability to communicate effectively in both oral and written communication
Capable of spending 10-25% of time lifting, carrying, moving items or patients weighing 50 pounds or more. Capable of constant standing and walking required during patient treatments
Knowledgeable in utilization of EMR; knowledge in EPIC is preferred
BLS certified
With experience in ventilator setting is preferred
Strong management skill
