Physical Therapist

This role will perform full physical therapy evaluation (collecting learner history, using standardized functional tests and outcome measures, determining PT diagnosis/prognosis, constructing learner-centered plans of care).
Ability to create home exercise programs along with learner education
Follow Individualized Family Service Plan (IFSP) and develop methodologies for reaching each childs specific developmental goals
Provide therapy services to improve gross motor development, address large motor concerns, and increase strength and muscle development
Provide therapy to assist with achievement of gross motor milestones at appropriate levels of development and ability
Ensure that progress is documented, and caregivers are active participants in intervention through addressing specific goals, coaching sessions, and communication with the entire team
