Occupational Therapist

• Responsible for providing occupational therapy services to assigned patients including evaluation, treatment planning, treatment programs, patient/caregiver education and discharge planning in accordance with the agency policies and procedures and the physician care plan. Assumes responsibility for managing patients; maintains timely communications with others involved in the care of the patient. Provides supervision to COTA, home care aides and family/caregivers involved in the care of the patient.

Graduate of an accredited Occupational Therapy program.

At least 1 year clinical experience as a licensed occupational therapist in an acute, rehab or skilled nursing setting.

Current Occupational Therapy licensure in the State of Ohio. Current CPR certification. Valid Ohio Driver’s license. Current auto liability insurance.
