- The Mercyhealth Autism Services team is dedicated to supporting patients ages 5-22 through individualized, one-on-one therapy. Our team works closely with patients to build essential skills, achieve personal goals, and foster growth through play-based interventions and engaging activities.
The Behavioral Health Case Manager MS - Outpatient works with the behavioral health multidisciplinary team to provide care for the patients admitted to inpatient behavioral health or mental health, addictions and child, adolescent day treatment programs. The Behavioral Health Case Manager MS would help with dispositional planning, care planning and group psychoeducation. The Behavioral Health Case Manager MS may also help with behavioral health consults in the ED and on the medical unit. Under the supervision and consultation of a psychiatrist the Behavioral Health Case Manager MS will gather data about patient condition and assist with disposition, pro viding resources and recommendations to patients in the ED or on medical floors. Performs other duties as assigned.
Responsibilities
Essential Duties and Responsibilities
- Consults and coordinates care of patient with the physician/psychiatrist, RN, social worker/therapist and other agencies.
- Assesses treatment planning and progress.
- Prepares treatment plan and goals for patients.
- Maintains pertinent patient data in the EMR in accordance with state and federal laws and hospital policy.
- Provides psychoeducational groups.
- Arranges for disposition and follow up recommendations.
Education and Experience
Master's Degree in a mental health or behavioral health field
Experience or field study experience in mental health site.
Working toward licensure is preferred.
Certification and Licensure
TACT certified within 90 days after hire.
Approved CPR/BLS within 90 days after hire.

Must be able to do all physical requirements of TACT and CPR.
