About the program: JFCAC has been awarded grant funding for the Older Adults Home Modification Program (OAHMP) by HUD. The OAHMP is a federally funded home upgrade program that is designed to help low-income seniors stay out of assisted living facilities by making home modifications which will make it easier for seniors to maintain their independence and perform daily tasks. This is a concept called aging-in-place.
Eligible low-income seniors will receive a comprehensive home evaluation utilizing the Home for Life Design Assessment tool. This will be performed at the clients' home by a contracted licensed Occupational Therapist (OT) or an in-house Certified Aging-In-Place Specialist (CAPS) who will be overseen by a licensed Occupational Therapist. The purpose of the evaluation is to catalog information and develop a scope of work for each client. The evaluation will be followed by licensed contractors completing the home modifications based on recommendations determined by the OT and/or CAPS.
Home modifications could include:
- widening doorways
- eliminating trip hazards
- installing grab bars
- upgraded lighting
- adding ramps
- other safety updates
Following the installation of recommended items a one-month follow-up evaluation will be made to ensure success of the initial set of recommendations made.
About the position: JFCAC is seeking a licensed Occupational Therapist to join our team in OAHMP as a contracted worker to complete the following duties utilizing the Home for Life design software:
- aging-in-place assessments
- follow-up assessments
- possible oversite of our in-house CAPS.
- evaluate data gathered by the CAPS
- approve or reject plans developed by the CAPS
Options for a one/other or hybrid contract are available.
Educational Requirements: Must be a licensed Occupational Therapist in the State of Missouri
Our goal: To assess and remedy 200 individual households for aging-in-place services over a 36-month period.
