Full-Time Occupational Therapist Liaison

If you are an Occupational Therapist looking for a different type of Occupational Therapy position, this opportunity located at St. Peter's Hospital is for you. This position is a member of the Complex Care interdisciplinary team and is responsible for ensuring patients transition to the next best site of care.

What you will do:

  • Directly co-treat patients in collaboration with the primary therapists, providing supplemental therapies.
  • Utilize your clinical expertise in occupational therapy to provide education and resources to ensure patients remain in the least restrictive environment when transitioning from acute care.
  • Serve acute care & complex care team, ensuring the no place like home approach is utilized in all appropriate scenarios.
  • Ensure timely progression to the next site of care by implementing appropriate occupational therapy treatment to vulnerable in need.
  • Responsible for educating case/care management teams, social work/utilization management services as well as the greater hospital system on functional status, DME options and alternatives to inpatient rehabilitation for appropriate patients in an effort to maximize community re-entry opportunities.
  • Problem solve and collaborate on the most vulnerable and complex SIOH needs in the hospital

What you will need:

  • Graduate of an approved Occupational Therapy program
  • Current NYS Occupational Therapist (OTR) license and registration
  • 1 year experience as Occupational Therapist required
  • 5 years in healthcare environment, including health plans, hospitals, ambulatory or post-acute care preferred.
  • Excellent verbal, written communication, and interpersonal skills.
  • Strong project management skills.
  • Ability to inspire and build confidence in others and to forge alliances and garner support.
  • Strong analytical and critical thinking skills, including the ability to resolve complex administrative, technical or human situations.
  • Anticipate and resolve a broad range of issues on multiple, sometimes unrelated, topics, to determine the need for process development and revision.
  • Familiarity with standard desktop and windows-based computer system, including email, e-learning, intranet and computer navigation.
  • Ability to use other software required to perform essential functions. Ability to utilize multiple electronic medical record platforms.

Pay Range:$35.85 - $45.96 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
