As a Contract Home Health Physical Therapist, you'll provide exceptional care to patients in the comfort of their own homes. Your mission, should you choose to accept it, will involve assessing patients, crafting personalized treatment plans, and delivering extraordinary interventions to help them regain their mobility, strength, and overall well-being.

Responsibilities:

• Conduct comprehensive initial assessments to evaluate patients' physical conditions, functional abilities, and mobility limitations.
• Develop individualized treatment plans based on assessments, patient goals, and physician orders.
• Educate patients and their families on the proper techniques for exercises, transfers, and the effective utilization of assistive devices.

Requirements:

• Possess a degree in Physical Therapy from a recognized school
• Hold a current state license
• Experience in Home Health and HCHB.
• Possess a valid driver's license and transportation
